- 1 Emergency exam for urgent dental issues during your plan year

- 10% Discount on all treatment completed at our practice (excluding Invisalign and teeth whitening)

Membership is for 1 year from date of enrollment
Membership dues are payable in full at the time of enrollment and are non-refundable.
Services must be completed within the membership year-benefits do not roll over
This is not a dental insurance and cannot be combined with insurance or any other offers.
Cancellations or no-shows without 48hr notice are subject to a $75 fee.
